Duties

The Senior Social Worker position is assigned to Social Work Services and the work is performed on the campus of the VA Long Beach Healthcare System (VALB), Long Beach, California. Senior social worker case manager utilizes a variety of advanced clinical skills adapted to both inpatient and outpatient clinic settings and community visits and outreach; incorporates the VA ICARE values within the position functions assigned and use advanced practice clinical skills for a specialized Veteran population where care is provided.Social workers functions include: brief psycho educational counseling or support, assists with inpatient and outpatient discharge planning needs, independently screen Veterans assigned for psychosocial needs via face-to-face or psychosocial/psycho-educational tele-health.Some of the duties include:

• As a member of a multidisciplinary team, assists in assessing the needs of Veterans assigned and document the social support services provided utilizing patient centered care approach to assure Veteran is involved in decisions regarding their healthcare.
• Participates in collaborative treatment planning with the Veteran, family and the multi/interdisciplinary treatment team; prior to team meetings, reassess the Veterans current psychosocial situation, to include financial, residential status, coordinate any equipment needs and other needs that may affect discharge.
• Provide high-risk psychosocial assessment in crisis intervention, seeking to address the cause as well as the presenting complaint; communicating with the consulting team, Veteran/family psychosocial resources available. Utilizes social work professional knowledge and experience as it relates to specialized Veteran medical and mental health diagnoses; disabilities and general treatment procedures.
• Assess for and provide care or case management appropriate to the assessed level of care.
• General knowledge of Veterans' benefits and services relating to special programs, service connected compensation, non-service connected pension. As consulted or requested, facilitates referrals based upon Veterans needs and eligibility.
• Utilize the VA electronic medical record for documentation.
• Utilize Event Capture code approved for social workers for inpatient care and uses Appointment Management/Encounters for outpatient clinics following the VA policies and processes for appointment scheduling.
• Documentation: the social worker provides a clinical assessment that supports the Veterans current situation or need based on services rendered in the encounter. Documentation codes utilized include diagnosis relate to either previous mental health or psychosocial related; use discipline approved CPT codes.
• Complete service and/or program specific documentation pertaining to caseload statistics and outcomes in program identified data bases.
• Respond and complete consults assigned.
• Completes VA specific clinical reminders within licensure level and social work scope of practice.
  Comply with VA and VALB policies and procedures.
• Knowledge and skill in the use of word-processing, data management and other computer systems (Outlook, CPRS), especially those programs in use by VA.

Shift: Monday-Friday 8:00am-4:30pm

This medical center operates 24 hours, seven days per week and you may be required to work days, evenings, nights, holidays, and/or weekends, to include modification in shifts and/or tours of duty, to meet medical center and patient care needs.

Per VHA Directive, all qualified social workers are responsible to provide after-hours on-call to the Emergency Department. Qualified social workers may volunteer for a calendar year on-call rotation schedule. In the event there are not enough volunteers, all qualified social workers will be assigned and rotated to the calendar year on-call schedule by the Chief, Social Work Services. Compensation is provided when after hour's on-call.

Qualifications

Basic Requirements:

• U.S. citizenship.
• Must have a Master's degree in Social Work (MSW) from a school of Social Work fully accredited by the Council on Social Work Education (CSWE). YOU MUST PROVIDE TRANSCRIPTS WITH YOUR APPLICATION.
• English Language Proficiency: In accordance with 38 U.S.C. 7402(d), No person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. You must be proficient in basic written and spoken English in order to meet the requirements of this position

Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website (http://ift.tt/1O3rgGW) to verify if that social work degree meets the accreditation standards for a master of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at an advanced level. Current state requirements may be found on the OHRM website. YOU MUST PROVIDE A COPY OF YOUR LICENSURE WITH YOUR APPLICATION.

GRADE DETERMINATIONS:
In addition to the basic requirements, applicants must have one year of experience equivalent to at least the next lower grade in the normal line of progression for the occupation in the organization. Examples of experience would typically include, but are not limited to:

GS-12 Experience, Education, and Licensure: GS-12 is above the full performance level grade for the GS-185 social work series and is used for senior social workers with more than two years of post-MSW experience and for social workers licensed at the independent practice level. YOU MUST PROVIDE TRANSCRIPTS WITH YOUR APPLICATION.
Examples of creditable specialized experience includes clinical experience which is demonstrated by: at least two years of post-graduate social work experience in acute/complex healthcare settings; utilizes shared decisions making skills with patients in developing individualized assessments, development of case management treatment plans and or coordination of care plan; knowledge, skill and advance practice experience working with adults who have a variety of acute or complex medical, emotional and/or psychosocial resource issues- specifically poly trauma/brain injury; advanced clinical social worker provides direct services, including interventions focused on interpersonal interactions, dynamics, and life management issues. knowledge and  skill in the assessment of domestic and intimate partner violence,  other forms of abuse and understanding of California mandatory reporting requirements; knowledge and/or experience working as a part of an inpatient or outpatient multidisciplinary team; demonstrates ability to be proactive in communicating between the patient/family and the multidisciplinary team; must demonstrate ability to work in a fast-paced clinical hospital/healthcare setting; all positions have minimum productivity and documentation standards; use of electronic medical record; work may be performed in both traditional and non-traditional office settings; skill in care coordination and care management, minimum of 1 year experience working with TBI/Polytrauma patients/Veterans; skill in working with interdisciplinary teams; strong administrative skills; experience working in VA and/or medical setting; and, experience working with chronic disabilities and knowledgeable of rehabilitative care and resources.
Licensure: Social Worker must be licensed or certified by a state at the advanced practice level which included an ASWB advanced generalist or clinical examination (except for licenses issued in California, which administers its own clinical examination for advanced practice). Two (2) years specialized clinical case management experience is in the field of healthcare social work (VA or non-VA experience) that demonstrates possession of current advanced licensed practice skills and judgment. If the incumbent possess certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and relates to functions of clinical supervision, submit certificates with your application. YOU MUST PROVIDE A COPY OF YOUR LICENSURE WITH YOUR APPLICATION. 

Healthcare work experience is required in knowledge and ability to perform duties identified in the announcement independently, including, but not limited to clinical evaluation and psychotherapeutic treatment interventions, crisis management, social work consultation, biopsychosocial assessments and evaluations. NOTE: A clinical healthcare setting is a medical center, comprehensive clinic (medical or psychiatric), where social work participates in collaborative treatment and is identified with the medical profession.

1.   Advanced knowledge of and mastery of theories and modalities used in the specialized treatment of complex physical or mental illness using evidenced based practice treatments. Ability to incorporate complex multiple causation in differential diagnosis and treatment of Veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. Ability to determine priority for services and provide specialized treatment services.

2.     Advanced and expert skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes experience brief therapy modalities for individual, group, and/or family counseling or psychotherapy and advanced level psychosocial used in the treatment of Veterans.

3.   Advanced knowledge and expert skill in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services. Ability to coordinate the delivery of specialized psychosocial services and programs. Ability to design system changes based on empirical findings.

4.   Ability to provide subject matter consultation to colleagues and students on the psychosocial treatment of patients treated in the specialty area, rendering professional opinions based on experience and expertise and role modeling effective social work practice skills. Ability to teach and mentor staff and students in the specialty area of practice and to provide supervision for licensure or for specialty certifications.

5.     Ability to expand clinical knowledge in the profession, demonstrating innovation in the creation of new models of psychosocial assessment or intervention to identify and address specialized clinical needs. Ability to write policies, procedures, and/or practice guidelines pertaining to the specialty population or specialty treatment program.
